Copyright Transfer and Author Consent Form

The Journal of Movement Disorders is pleased to publish your manuscript # _____________________ ,

  1. I (We, the undersigned coauthors of this manuscript) have contributed to the (1) data design, analysis, or interpretation, and the (2) writing or critiquing of drafts of the manuscript. We approve the submitted version of the manuscript.
  2. I (We) warrant that this manuscript is an original work and has not been published elsewhere in whole or in part in any language (except in abstract form). We are jointly responsible for the release of any or all of the material contained within the manuscript.
  3. I (We) also declare that research involving human subjects was performed in compliance with the principles of the Declaration of Helsinki (1964), and that the involvement of animals was approved by the applicable institutional ethics committee or review board and conformed with generally accepted guidelines governing such work.
  4. I (We) further attest that we have herein disclosed any and all financial and other relationships that could be construed as conflicts of interest, and that all sources of financial support for this study are disclosed in the manuscript.
  5. I (We) warrant that this manuscript contains no violation of any existing copyright.
  6. Upon acceptance by the Journal of Movement Disorders, all copyright ownership for the above-named manuscript is transferred to the publisher (Korean Movement Disorder Society).
  7. I (We) agree that the publisher (Korean Movement Disorder Society) may; 1) publish the manuscript in the Journal of Movement Disorders on its own, or with other related material, 2) publish the manuscript in multi-contributor book form or other edited compilations by the publisher, 3) reproduce and/or distribute throughout the world in printed, electronic or any other medium and to authorize third parties (including reproduction rights organizations) to do the same, and 4) use any images from the manuscript on the cover of the Journal and in any marketing material.
  8. The publisher and I (we) agree that anyone may copy and post the PDF version of the manuscript online, unless used for commercial purposes or against the interest of the publisher or authors.
  9. The publisher and I (we) agree that 1) no one can re-use the manuscript in whole or in part before they obtain permission from the publisher, and 2) any permission fees will be retained by the journal.
  10. All requests to adapt substantial parts of the manuscript in another publication will be subject to the authors’ approval (which is deemed to be given if we have not heard from you within 4 weeks of our writing to you at your last notified address to obtain your approval).
